What this policy covers

This policy explains what information Healthy Living Option Inc. (“HLO Inc.”, “we”, “us”) collects through this website, how we use it, and the choices you have. It covers this website only. It does not describe how we handle records of individuals who receive our services. Please ask us about those practices.

Information you give us

When you use a form on this site, we receive what you type into it. Depending on the form, that can include:

• your name, phone number and email address, and how you prefer to be contacted;
• your relationship to the individual who needs help, and the individual’s first name or initials, county, and answers about DDA eligibility, funding priority category, Person-Centered Plan status and services of interest;
• for referrals, the referrer’s role and agency, and what you choose to tell us about the individual’s situation and the services needed;
• for job applications, your contact details, the position you want, your resume and your message.

Please do not send sensitive documents

Please do not send Social Security numbers, medical records or other sensitive documents through the forms or by ordinary email. We will arrange a secure way to collect anything we need.

Information collected automatically

Like most websites, the servers that deliver this site receive your IP address, browser type and the pages you request. We use this only to operate and protect the site.

To stop automated spam, our forms use hidden checks, record when a form was opened, and store your IP address with your submission. For up to 24 hours we also keep a one-way scrambled (hashed) version of the email address used, to limit repeated submissions.

Cookies and your browser

This site uses one strictly necessary cookie, described below. It does not set advertising or tracking cookies, and we do not use analytics.

Your browser also remembers your light or dark theme choice, and that you closed our cookie notice. These stay on your device and are never sent to us.

Security check by Cloudflare

Our forms use Cloudflare Turnstile to check that a person, not an automated program, is sending them. Turnstile processes technical information about your browser and device for this purpose only, under Cloudflare’s privacy policy (cloudflare.com/privacypolicy).

Third-party services

• Maps. The map on the Home and Contact pages is provided by Google Maps. When it loads, Google receives your IP address and browser details, and its privacy policy applies.
• Email. Messages from our forms are delivered by our email provider, which may use them only to deliver them to us.
• Resumes. Every resume sent with a job application is checked for viruses by an automated scanning service, which scans it in memory and does not keep a copy.
• External links. Links on the Resources page go to other organizations’ websites. We do not control them and are not responsible for their privacy practices.

How we use information

• to respond to your questions, requests for services and referrals;
• to consider job applications;
• to keep the website secure and working properly.

When we share information

We share information only as needed to respond to your request, with service providers who help us run this site and receive form submissions, and when the law requires it. We do not sell your information.

Job applications

We use what you send with a job application only to consider you for that role and similar roles at HLO Inc. Resumes are stored encrypted, are never sent by email, and only HLO Inc. staff responsible for hiring can open them. Resumes are deleted automatically three months after an application arrives; the rest of the application is kept with our hiring records.

How long we keep information

We keep website messages and requests only as long as needed for the purpose they were sent and to meet our legal and record-keeping obligations. After that, we delete them or make them anonymous.

How we protect information

We use technical and organizational safeguards to protect personal information, including encrypted connections, limited staff access and a record of who opens what. No website or system can be guaranteed completely secure, but we work to keep your information safe.

Your choices

You can ask us what information you have sent us through this site, or ask us to correct or delete it, by contacting us below. The law may require us to keep some information. We may need to confirm who you are before acting on a request.

Children

This site is written for adults and for the families and professionals who care for them. It is not directed to children under 13.

Changes to this policy

If we change this policy, we will post the new version here and update the date above.
